- Download and print this form (requires latest version of Adobe Acrobat Reader ), and turn it in to the office.
- Phone: 206-242-1239 and reserve your spot with a credit card over the phone.
- Fax: Complete the Registration Form and fax your form to 206-242-8363.

Advance payment of fee and tuition holds your
student's place in summer classes.

- $5 Registration fee per new student
- $10 Max per family
- Current MMDA students who enroll in Summer Session will have the registration fee waived!

Advance payment of registration fee plus one month tuition holds your student’s place in class.

Registration Fee for Fall through Spring |
- $20 Registration fee per new student
- $15 Registration fee per returning student
- $40 max registration fee per family
- $5 discount per child if tuition is setup on auto-pay

Tuition Discounts |
- 5% if taking 3-4 classes per week
- 10% if taking 5-6 classes per week
- 15% if taking 7 or more classes per week
- Refer-a-friend and receive $10 off your next month's tuition!
- Home school students are eligible for discounted tuition
- Families with limited income may contact our office for information about tuition scholarships
- Fall Tuition Special:
-Pay your tuition in 2 installments (September and February) and receive a 5% discount on tuition
-Pay your annual tuition in full within 30 days of registration and receive 5% discount on tuition and 10% discount on season finale performance fees (expires 11/1/09)
- Auto-Pay Discount:
Set up your account for direct withdrawal payments and receive $5.00 off registration fee per child
- All discounts are cummulative
